PRP – Adults

The PRP – Adults program at SBHS is the rehabilitation arm of SBHS that provides services to adults (18 and above) with severe mental health issues who need support to build coping skills in their day-to-day activities.

SBHS shares information with the persons served, appropriate consent to families, referral sources, payers/funding sources, other relevant stakeholders and the public through its various brochures in its waiting area, notice board and information on its website. Our scope of services will be revised on a yearly basis.

Population Served

The PRP program caters to adults, adolescents and children with severe mental health issues to developing day to day coping skills.

Our Service Settings

We provide services in various settings including both onsite and offsite (blended) services. Our philosophy is to meet our clients where they are and when they are best comfortable.

Psychiatric Rehabilitation Assessment – Upon admission to the program, trained rehabilitation staff meet face to face with the individual to complete a functional skills assessment, designed to identify each individual’s strengths, skills and needs.

Individual & Group Rehabilitation Activities – are designed to reduce functional behavioral impairments and develop and/or restore age-appropriate skills in a variety of areas, including:

  • Self-Care Skills
  • Semi-Independent Living Skills
  • Social/Interactive Skills
  • Anger Management Skills
  • Education
  • Symptom Management
  • Accessing Community Resources
  • Personal Safety
  • Maintaining Age-Appropriate Boundaries
  • Time Management

Adults and their family members choose the services they want to receive (onsite, offsite or both) and the location where they want to receive them. Services are provided based on a monthly calendar of activities that typically include a combination of onsite and offsite services. Monthly program calendars are provided to all children and their families. Rehabilitation Counselors schedule visits with the individuals based on the level of need and client preference and availability. Individuals who have chosen to participate in both onsite and offsite services participate in a minimum of four visits/services per month (2 onsite activities and 2 offsite visit or 1 onsite activity and 3 offsite visits). Individuals who only elect to receive onsite or offsite services (not both) participate in at least two activities/visits per month. Additional visits are scheduled based on the Individual’s needs and availability.

Hours of Services

Our hours of operations are 9am – 6pm, Mondays to Fridays and we provide periodic trainings on Saturdays as needed.

Frequency of Services

Services are provided based on medical necessity of each client served. Although our payer source has capped visits/contacts to 6 times per calendar month for adult and 4 times per calendar month for adolescents and children, however our services are provided beyond this minimum contact where clinically necessary.

Payer Sources

SBHS accepts State of Maryland Medicaid clients or Medical Assistance only.

Fees

Our PRP Program currently accepts Medicaid clients/Medical Assistance only and may provide self-paying options and sliding scale.

Referral Sources

The referral source for our PRP program is majorly from outpatient mental health programs we collaborate with. We also accept referrals from licensed clinicians, DSS, DJS, etc. Each referral must first be assessed by a licensed clinician or therapist prior to enrollment in a rehabilitation program. There are COMAR regulated diagnosis to qualify for PRP services, and as such, clients will need to meet this threshold for referral for services. The PRP program will accept referrals as long as these are from licensed clinicians and the required diagnosis are met.

Intake Criteria:

To be eligible for the rehabilitation services in SBHS, clients must meet the following:

  • Have a health insurance plan with Maryland Medicaid.
  • Be active in outpatient mental health treatment.
  • Meet designated mental health diagnosis for PRP services.
  • Be referred by a licensed mental health practitioner such as a therapist or a clinician.

Transition Criteria:

Clients in the SBHS PRP program can be transitioned for the following reasons:

  • Attainment of or a material change in a client’s individual rehabilitation goals.
  • Clients may be transitioned to a higher or lower level of care based on the periodic review of their individual rehabilitation plan with their case manager.

Discharge Criteria:

Clients in the SBHS PRP program can be discharged for the following reasons:

  • Attainment of their individual rehabilitation goals.
  • Discharge from mental health treatment is an automatic criterion for discharge as clients are unable to remain in rehabilitation programs without treatment.
  • Clients are no longer eligible for services.
  • Client no longer requires or wants services.
  • Client requests transfer to another provider.

  • Referred by a Licensed Mental Health Professional
  • Diagnosed with a behavioral health diagnosis
  • Willing to participate in PRP services
  • A Medical Assistance recipient

Step 2. Therapeutic Interaction (one on one sessions with the therapist)
Step 3. Psychiatric Evaluation (initial session with the psychiatrist)

SBHS will not discriminate against any person receiving health services because of their inability to pay for services or based on race, gender, color, nationality, disability, religion, or sexual orientation.

A client admitted to SBHS must be between the ages of 6 and 100 years old. SBHS admits children, adolescents and adults with problems, which causes conflict within the family, school, and community.

  • Department of Social Services
  • Baltimore City Court System
  • Department of Juvenile Services
  • Parents or Guardians
  • Other Health Care Facilities
  • Schools/school systems
  • Self-referral
